REQUIRED LISTS AND FORMS
As for any emergency, prepare in advance all the lists and forms you'll need. You will have many other things to do when the problems start and the last thing you'll want to be doing is creating multiple copies of multiple forms and lists.
Furthermore, you may be required by the local authorities to keep certain records should staff fall sick in the workplace.
Forms can be created well in advance using the guidelines outlined below. You should then print out enough copies to meet your needs and store them somewhere safe and out of the way for when the time comes.
Alternatively, Bird Flu Manual Online has created customizable templates of each of these forms which are available for purchase at only US$4.95 per template. It is recommended as a minimum you create:
A copy of the Influenza Notification Form is given to each employee. It outlines the symptoms to look for and explains the procedures staff should go through should they feel ill at home and at work. It contains the contact information of the Influenza Manager and instructs them to notify him/her for advice and notification.

The Infection at Work Record Form is used to keep a record of staff who fall ill at work. It records the symptoms felt by the staff member, their contact details, personal information and recent travel history.

The Contact Tracking Form records who sick employees have been in close contact with, including name, address, phone number and email address.

The Emergency Contact Phone List contains contact information of critical managers and staff. All staff members should be given a copy of the Emergency Contact Phone List, and not just those on the list. It makes good sense to populate this form now as it would be useful in any type of emergency. It should also be made into a pocket size document that staff members can keep on their person at all times.

The Travel Application Form is designed to append to your existing travel application form if you have one, but should also work as a stand alone if you don't. It records where employees are traveling to on business or holiday, and keeps contact details for each leg of their journey. If staff are traveling to an infected area, the form can be used in conjunction with any special travel policies in force at the time.
